Hehe, I played a bit of Xbox Live when I got my original Xbox. It really depends on the group you're playing with - there's always one person like that, but if the majority of people in the given "session" are normal they usually shut them down.
Also, on Live you can "report" people being especially obnoxious, which I think helps keep things somewhat under control.
