Apparently it's debated how frequent anesthetic awareness acutally is, although not surprisingly, the movie exaggerates both the frequency & the experience...

Quoted: What makes Awake a bit more annoying than most medical jaunts is that, unlike its peers, it claims a noble and self-righteous pedigree: As the first exposé of a previously covered-up, denied, and miserable condition called "anesthetic awareness."

Puffing Away That PG Rating - New York Times

Quoted: Smoking would be considered alongside sex and violence in assessing the suitability of movies for young viewers.
